You pay when you come to the appointment. You can pay cash, debit with a Maestro bank card or with a credit card (Visa or MasterCard).

1 How do I sign the list of questions?
By checking at the bottom of the form that you have filled in everything truthfully. We will then receive your list of questions.
2 I have no contact with the other parent of my child. How should I sort out the consent aspect?
For a passport for children up to the age of 18 or for an ID card for children up to the age of 12, both parents must give written consent. Is this not possible? Mail us via: publieksdiensten@bergenopzoom.nl
3 Do both parents have to come to the appointment for a child?
No, this is not necessary. However, both parents have to give written consent. Please bring the completed form to the appointment. Also bring a copy of the passport of both parents.
4 Does my child have to come with me to the appointment?
Yeah. The child must also be present at the pickup or delivery.
5 I made the appointment, but I did not receive an e-mail. Have I made an appointment now?
You can only be sure when you have received an e-mail. If the mail is not in your spam box either, then something went wrong. Then send us an e-mail: publieksdiensten@bergenopzoom.nl.
